Barrett’s esophagus is a premaligant precursor that predisposes to the development of esophageal adenocarcinoma. Non-dysplastic metaplasia in Barrett’s esophagus can progress to low-grade dysplasia, high-grade dysplasia, and finally to adenocarcinoma. Although the incidence of adenocarcinoma in patients with Barrett’s esophagus is low, surveillance is justified because the outcome of adenocarcinoma is poor. A 56 year-old male patient presented with complaints of dyspepsia. Barrett’s esophagus with irregular nodular lesions was observed endoscopically and it was further demarcated after methylene blue staining. The biopsy findings were consistent with Barrett’s esophagus, dysplasia and adenocarcinoma. Superficial adenocarcinoma was confirmed after esophagectomy. We report here a case of esophageal adenocarcinoma associated with Barrett’s esophagus. (Kor J Neurogastroenterol Motil 2006;12:81-86)
